BHSD CJS (Full Service Partnership)
The Behavioral Health Services Department (BHSD) Criminal Justice Systems (CJS) Full Service Partnership (FSP) programs are carried out through a variety of contracts with Community Based Organizations (CBOs). Individuals served are high-risk and high level need, and frequent users of involuntary care. These programs offer intensive, comprehensive outpatient services, which include mental health treatment (individual and group therapy), medication support services, crisis intervention, and case management services. Clients must be referred by BHSD Treatment Court and the BHSD RRC. Slots are available across the two types of programs: AB109 and MHSA.
Eligibility and Referral Process
Criminal justice involved individuals with serious mental health and/or co-occurring conditions; residents of Santa Clara County; must meet Medi-Cal medical necessity criteria; have Medi-Cal based in Santa Clara County or be uninsured; have three or more psychiatric hospital admissions within the last year; and may have histories of hospitalizations, institutionalization, poor adherence with medication treatment, and difficulties in participating in structured activities.

BHSD CJS Aftercare Program
The intent of the BHSD CJS Aftercare program is to provide behavioral health services to higher functioning criminal justice involved individuals with mental health & co-occurring conditions. Individuals served would be nearing graduation or have graduated from the criminal justice system within the last six months but continue to require behavioral health services. Services are aimed at reintegrating clients into the community and reducing recidivism and include: mental health treatment, case management services, medication support services and crisis intervention.
Eligibility and Referral Process
Criminal justice involved clients with serious mental health and/or co-occurring conditions; adults and older adults who are residents of Santa Clara County. Individuals must meet Medi-Cal medical necessity, have Medi-Cal in Santa Clara County or be uninsured. Individuals are referred by the BHSD Treatment Court staff and BHSD RRC.

BHSD CJS Crisis Residential
BHSD CJS Crisis Residential Services is an alternative to acute psychiatric units and locked psychiatric treatment facilities for criminal justice involved individuals who would otherwise require hospitalization, and do not have medical complications requiring nursing care. This program is a 24 hour crisis residential treatment. Service activities include mental health services such as individual and group therapy, case management, crisis intervention, and medication support. Individuals served must be referred by the BHSD CJS Division, BHSD Reentry Resource Center, or the BHSD Treatment Court.
Eligibility and Referral Process
Individuals served must be high risk, criminal justice involved adults and older adults who are at risk of decompensation or hospitalization but do not meet 5150 Criteria; do not require nursing care services and who have severe mental health and co-occurring (mental health and substance use) conditions; be residents of Santa Clara County; have Medi-Cal benefits in Santa Clara County or be uninsured.

Evan's Lane Outpatient & Residential
The Evans Lane Wellness and Recovery Center has outpatient and residential programs dedicated to serving adults diagnosed with severe mental health and co-occurring (mental health and drug & alcohol) conditions who are involved with the criminal justice system. The program works in collaboration with the Santa Clara County Superior Court, the Santa Clara County Adult Probation Department, and the California Department of Corrections and Rehabilitation.
There are currently 150 outpatient slots and 56 residential slots.

Criminal Justice client must have a mental health diagnosis; be residents of Santa Clara County; meet Medi-Cal necessity; have Medi-Cal assigned to Santa Clara County.

Reentry Resource Center Behavioral Health Team (assessment/service linkage)
The Behavioral Health Team at the Reentry Resource Center (RRC) is one of the primary screening and referral portals to clinical treatment for reentry clients. The team provides SUTS outpatient treatments at the RRC and functions as a gateway to behavioral health services by linking clients to Substance Use Treatment Services (SUTS) and Mental Health treatment, as well as other reentry services.

Santa Clara County adult Criminal Justice clients. Clients can walk-in at the Reentry Resource Center and request a BHT assessment.

Recovery Café
Recovery Café in San Jose is a healing community for those who have been traumatized by addiction, homelessness, and mental health challenges. The Café provides the kind of resources and relationships that provide stability and healing. The Café provides:
Recovery Circles: small weekly peer support and accountability groups.
School for Recovery: 8-week courses in recovery/addiction, life skills, inner healing, and healthy living.
Those interested in membership can come on Monday's at 10:30am to do an orientation in-person.
Members are expected to join a Recovery Circle that they must attend once a week. Members can only come to the Café the day of their assigned circle. They can come in as early as 10:30am for latte hour and attend a circle either at 11:30am or 1pm.
Members can also order a lunch before 12.
Members can stay inside the Café after group and hang out until 3pm the day of their circle.

Blackbird House
A peer-run respite home for Santa Clara County adults experiencing a mental health crisis. Stays at Blackbird House are intended to prevent unnecessary hospitalizations and they empowers adults to move toward improved wellness, resilience and independence. Blackbird House provides voluntary, short-term, non-clinical crisis support for up to 14 days.
Eligibility and Referral Process
Eligibility requirements:
• 18 years or older
• Resident of Santa Clara County
• Either enrolled in Medi-Cal, be eligible for Medi-Cal or have no health insurance
•Self identify as experiencing mental health concerns or distress that do not meet requirements for inpatient hospitalization •Have an identified place of residence to stay in Santa Clara County at the time of intake (which may include a shelter)
•Able to manage medical needs independently
•Voluntarily agree to engage in services
•Not in need of detox or immediate medical attention
•Can function in communal environment independently
