Faith-Based Resource Centers

The four centers provide a warm welcoming environment to individuals reentering the community from incarceration. Clients can receive wrap-around services through case management. The centers assist clients with a wide variety of services and resources, as well as faith-based support and spiritual wellness. Services such as employment, food, clothing, connection to spiritual community, recovery programs, shelter information, family reunification, assistance obtaining a CA identification card/driver license, and mentoring programs.

Eligibility and Referral Process

Clients must be justice involved adults (ages 18 to 59 ) and older adults (60 and over) who are exiting incarceration and returning to the community within the last 12 months and/or who are involved in the criminal justice system (e.g., Probation, Parole, Pre-trial, etc.). Individuals must be residents of Santa Clara County. Interested clients need to visit the Reentry Resource Center and be referred by Behavioral Health Faith Based staff or can visit one of the Faith Based Centers as a walk in.

A one-stop shop center than provides clients who have been released from jail/prison within one year a variety of services such as clothing, housing, employment, medical services etc.

MHS - LINKED

MHS LINKED Program supports individuals who have recently reentered the community. The primary goal is to assist reentry clients in navigating and accessing available resources in the Adult Reentry Network and larger community, and by extension, to facilitate reentry into the community and attainment of self-sufficiency. Resources will address employment, housing, transportation, clinical, ongoing, and immediate needs. 

Eligibility and Referral Process

Clients need to visit the RRC in person and register for a referral. To be eligible, RRC client must be released from custody within the last thirty (30) days, approved by RRC staff, unemployed and homeless.

San Jose State University Service Navigation 

In custody: SJSU-SN supports individuals prior to release or who have been recently released through service navigation case management. Intensive services will be concentrated during the critical first month of release as clients stabilize, by screening clients’ immediate needs and creating post-release plans, while in custody. Immediate needs may include housing, employment, mental health, food, clothing, transportation, including rides to pick up clients as they are released.

Community: SJSU-SN will assess individuals’ needs and assist in eliminating barriers to self-sufficiency through planning (stabilization assessment) and accessing resources. Direct support services provided are assistance with employment, housing placements/SLE fee assistance, mental health/substance use support, and more.

Ineligible referrals can receive a one-touch service linkage, one-time assistance to immediate needs (e.g. Lyft ride, VTA tokens, hygiene kit).

Eligibility and Referral Process

In Custody: Clients must be incarcerated at County Correctional Facilities and have release dates within 45 days or less and have spent at least 30 days in custody by the release date. Clients must be on track to be unemployed at release with no permanent housing option in place. Rehabilitation Officers, Custody Health, or other DOC staff can send referrals via email. Upon release, clients need to visit the RRC and register for a referral.

Community: Must be RRC clients who have been released from custody (jail/prison) within the last thirty (30) days, approved by RRC staff, unemployed and with no permanent housing.

Never Give Up Program offered by Breakout Prison Outreach

This program is designed to assist clients in navigating and accessing available resources in the Adult Reentry Network and larger community, and by extension, to facilitate reentry into the community and attainment of self-sufficiency.  Services are offered to individuals who are in custody and those who have been recently released from custody.  Eligible clients are required to complete an intake and assessment prior to receiving services. 

Eligibility and Referral Process  

• 18 years or older
• Resident of Santa Clara County
• All clients must be homeless and unemployed.
• In-custody clients must be incarcerated at County correctional facilities and have release dates within 45 days or less and have spent at least 30 days in custody.
• In community clients must have been released from  custody within the last six months.

In-Custody Referrals:  
Clients will receive one-on-one visits to create a discharge plan addressing basic needs for the first 24-72 hours following release.  Transportation, housing, emergency clothing, hygiene, food, and identification needs will be included in the discharge plan.   

In Community Referrals: 
Clients will be assisted with service navigation support through case management and individualized care plans, weekly support groups and monthly prosocial activities.

Dept. of Child Support Services

The Dept. Of Child Support Services can assist with the following:

  • Establishing and or enforcing an existing child support order
  • Parentage establishment
  • Driver’s license suspension due to non-payment of support
  • Was support charged during incarceration?
  • Modifications of your support order
  • Debt Reduction Program
  • ​​​​​​​Adding your name to your child’s birth certificate
